Minnesota Delegation of Power by Parent formPursuant to Minnesota Statute § 524.5-211, a parent or guardian can delegate power of the health and welfare of their children to another third party for up to one-year.

I have been asked by several clients about issues related to this form and possible guardianship of minor children or incapacited persons.  Many of the people who call me do not realize that their is a form available that must be used to delegate such power.
